Checklist of Forms For Confirmed Staff
(This list includes all steps that must be completed before offer of employment is finalized, and before you will be allowed to begin your employment at camp)

If you have been offered a position at YMCA Camp Hi-Rock, you can use this page to download and print any documents that will complete your hiring packet. You will need Adobe Reader to open these files. Adobe Reader is free and can be downloaded at the Adobe website.

Forms that can be emailed should be sent to paperwork@camphirock.org, or directly to the Director of Administration: admin@camphirock.org. Only certain forms can be emailed; please read the instructions carefully.

Please note the following:
1. The forms available on this page are the same ones you would receive if you requested them through the Camp Office. Please contact us immediately if you have additional questions.
2. Some of these forms are required as SOON as you receive an offer of employment. ALL of these forms are necessary before you will be allowed to begin work at camp. It is possible to take care of all your paperwork before you begin staff training!

• Complete Before Arrival (Downloadable version) (steps you must complete before you arrive at camp - all forms can be mailed in advance, saving you time upon arrival)

• Notice and Authorization Concerning Consumer and Investigative Reports (Central Connecticut Coast YMCA Background Check) Mail/Fax/Hand-deliver IMMEDIATELY
• Local Criminal Background Check (International Staff) Mail/Fax/Hand-deliver IMMEDIATELY
• MA CORI Request Form Email/Fax/Hand-deliver IMMEDIATELY
• Central Connecticut Coast YMCA Application for Employment Mail/Hand-deliver
• Minimum of 5 Years Employment History
• 3 Written References Mail/Hand-deliver

Steps Needed Before Arrival - Must Be Completed Before You May Begin Working

• YMCA Camp Hi-Rock Medical Form and Medical History Form
• Medication Administration Release Form (only if younger than 18)
• Certification Documents (any applicable)
• Working Papers (only if younger than 18)
• Copy of Driver's License (if applicable)
• Signed Job Description (contact the Hi-Rock staff member who offered you employment)
• Terms of Employment (contact the Hi-Rock staff member who offered you employment)
• Federal W-4 (Americans only)
• Massachusetts M-4 (Americans only)
• Personnel Policy and Code of Conduct Receipt (please download the Personnel Policy and Code of Conduct)
• Web Code of Conduct and Receipt
• Federal I-9 Employment Eligibility Form (Americans only; complete section I only)
